Family Favorites : Voting Form

Tell Us Your Family’s Favorite Places

Help us recognize the best family spots in the Triangle by casting your vote for the Carolina Parent Family Favorites Awards.
We'll share the top choices in our July 2011 issue and online.

Ballots with at least 20 categories completed will be entered into drawings for family-friendly prizes including:

One-year family membership to the North Carolina Zoo
A family four-pack admission to the Museum of Life and Science
Gift certificate for $100 to Skin Sense A Day Spa

Note: One ballot per person. Anonymous ballots, ballots that are filled out and then photocopied, or ballots naming only one establishment will be disqualified. Ballot stuffing is not allowed.
